As a LOX Professional or Stylist, the purchaser hereby claims exemption on the purchase of tangible personal property and selected services made from LOX Extensions LLC. This will serve to certify that this claim is based upon the purchaser’s proposed use of the items or services, OR the status of the purchaser. All items purchased are for resale at retail. It is the Purchaser’s responsibility to ensure the eligibility of the exemption being claimed. All claims are subject to audit. Non-qualified transactions are subject to tax, statutory penalty and interest.
The buyer declares, under penalty of perjury, that the above information is true, that you have consulted the statutes, administrative rules and other sources of law applicable to the exemption, and that you have exercised reasonable care in assuring that your claim of exemption is valid under Michigan law. ORDERS
We request information from you on our order form. To buy from us, you must provide accurate contact information (like name and shipping address) and accurate financial information (like credit card number, expiration date). This information is used for billing purposes and to fill your orders. Please make sure that you enter the correct billing address and contact information as it appears on your bank or credit card statement. Transactions may not go through if the information entered does not match exactly what your bank or credit card provider has on file. Some transactions may also be held for further verification by our customer experience team.
